The DECLARE-TIMI 58 trial enrolled a majority of patients who did not have established atherosclerotic cardiovascular disease (ASCVD), and did not show significant reduction in MACE.19 This may imply the cardioprotection offered by SGLT2 inhibitors is less evident in such a group of patients .18 However, analysis from the EMPA-REG OUTCOME trial reported consistent benefits regardless of baseline risks and prior history.21 Neither of these trials was designed to investigate HF outcomes, but they illustrated the value in identifying patient subgroups that benefit most from SGLT2 inhibitor treatment
